Social Worker Officer Job in Marsabit Kenya - Food for the Hungry / CCSMKE

FH / Kenya (Food for the Hungry, Kenya) and Christian Community services Mt. Kenya (CCSMKE) are both Christian organizations partnering to offer Care and treatment to PLWHIV in Marsabit and Isiolo districts. Both organizations have established a comprehensive care centers in Marsabit – Tumaini Medical Centre and in Isiolo - Tumaini Isiolo Clinic that are giving care and treatment to PLWHIV while offering affordable medical services to general population in Marsabit and Isiolo.
Through this partnership CCSMKE seeks to fill in the following vacant position.
Position: Social Worker Officer (Based in Tumaini Clinic -  Marsabit)

Reporting To: Senior Nurse

Specific Duties

Screen patients for HAART protocol  and social supportComplete Social Work Screening form for patients in the HAART ProtocolConducts home assessments for patient’s lost to follow up and those requiring a home visit.Maintain accurate and up to date records of lost follow up, in coordination with the data officer. Responsible for the management and tracking of all cases which are lost to follow up for the purpose of reducing the amount.Identify and build relationships with various HIV/AIDS support, home based care and social service organizations throughout Kenya, maintain records of such relationships and make patient referrals accordingly. Work with Nutritionist to identify most needy cases for supplemental nutritional supportWrite reports with respect to department activities weekly, monthly or as requiredCoordinate posttest club activities and facilitate weekly meetings, in coordination with counselors.Qualifications / Experience (Minimum)Diploma in community health, social worker, community worker, counseling, or aboveMinimum of 2 years in community work especially on HIV and AIDS programsTrained on HIV  and AIDS Management, VCT/HTC, Computer literate especially MS Word and MS ExcelMust be able to speak any of the local languages in Marsabit county.Interested and qualified candidates should send their applications, updated CV with three referees (clearly indicating their current or expected salary), day telephone contacts to:  or email address hr_fhkenya@fh.org.  Closing date is 16th November, 2012. Short listing will be done as CVs are received.
