SHOPS Project Kenya - Capitation Quality Assurance / Monitoring & Evaluation Consultant
Purpose of Assignment: The purpose of the assignment is to provide technical assistance in the development and implementation of a monitoring and evaluation (M&E) Plan and monitor the insurer, provider and client perspectives for a capitation pilot in a health insurance company. The insurer intends to switch from a fee-for-service (FFS) reimbursement system to a capitation mode of payment for their corporate medical products covering out-patient care.
 Rationale / Justification for Assignment: Since FY 2013, SHOPS Kenya has been offering TA to an insurer on building their capacity for marketing and distribution. As continuation of this technical assistance, SHOPS Kenya will support the insurer as it pilots a capitation model of paying their network healthcare providers. Brief Background: In FY2014, SHOPS will continue to provide technical assistance to the insurer to facilitate the roll-out of their health insurance product in accordance to the distribution as well as marketing strategies. In addition, SHOPS will support the insurer design and pilot a capitation mode of payment.
Introducing prospective provider payment mechanisms such as capitation controls the negative provider incentives associated with FFS and presents a significant opportunity to control claims ratios
and administration costs for insurers. On the provider end, prospective payments give the advantage of pre-negotiated payments with a lower administrative burden and a predictable revenue stream. Capitation payment systems also allow providers to spend more time on patient care, which frequently increases patient and provider satisfaction.
This being a new concept, the insurer intends to monitor and document the capitation pilot to learn from the experience as they transition from FFS to capitation for all their clients, and support the improvement of the capitation model in subsequent iterations. SHOPS therefore intends to support the insurer with technical assistance to develop systems to identify indicators for continuous monitoring of insurer, provider, and client perspectives. SHOPS will also assist the insurer to develop systems that identify and mitigate challenges that arise during the pilot. SHOPS researchers will assemble this information in an end-term evaluation of the capitation pilot that may be used to advocate for future policy changes.
1. Under the guidance of the SHOPS capitation research lead, support the insurer to identify the challenges associated with capitation and indicators for monitoring the capitation pilot. This will include (but not be limited to) collecting data on the following:
a. Insurer: utilization data, claims incidence, diagnoses, claims ratios, administration costs, challenges and recommendations for improving capitation, comprehension of capitation, and other information relevant to informing rate negotiations with healthcare providers.
b. Providers: satisfaction and motivation in response to the capitation pilot, administration costs (at the beginning and through the duration of the pilot term), profits and sustainability of capitation payments (relative to FFS), challenges and recommendations for improving capitation, comprehension of capitation, and other information relevant to informing rate negotiations with insurance providers.
c. Clients: patient satisfaction, out-of-pocket spending, wait times, access and utilization of needed care, perceived coordination of care
2. Train insurers, providers, and survey teams to implement monitoring tools for use during the capitation pilot. This includes an audit checklist/tool for insurers that will ensure that providers deliver high quality services to clients, a customer satisfaction monitoring tool for a survey of capitated clients, and a survey for health facility administrators and providers around their experiences with participating in the capitation pilot.
a. Coordinate the collection of survey data through phone or facility interviews.
b. Manage a short-term team of data collectors to implement the survey.
3. Assist the SHOPS capitation research lead to develop a quality assurance/monitoring and evaluation plan for use during the capitation pilot and beyond the pilot as the insurer transitions from FFS
4. Implement a training program for the insurer managerial staff on how to use the indicators identified, tools designed and the quality assurance/monitoring plan.
5. Support the insurer in the monitoring of these indicators during the capitation pilot.
6. Provide inputs to an end-term evaluation of the capitation pilot including a report to document findings, lessons, successes and challenges (from insurer, provider, and client perspectives).
 Key Deliverables & Expected Outputs:
1. Identify indicators, design tools and develop a plan to monitor the capitation pilot along the insurer, provider and client perspectives
a. While overseeing a team of data collectors, the consultant will be responsible for the creation and cleaning of the following datasets (at baseline, mid-line, and endline):
i. Patient satisfaction survey
ii. Insurer satisfaction survey
iii. Provider satisfaction survey
2. Training of the insurer’s team on indicators and tools designed to monitor the pilot.
3. During the capitation pilot, the consultant will provide ongoing supervision and feedback to the insurer’s team for monitoring the pilot and quality assurance.
4. An end term evaluation that will document the pilot’s lessons, successes and challenges for future learning as the insurer fully transitions to capitation.

Background Strengthening Health Outcomes through the Private Sector (SHOPS) Project is a global five-year USAID-funded project, which aims to expand the role of the private sector, including NGOs and for-profit entities in the sustainable provision and use of high-quality health information, products and services.SHOPS builds upon decades of USAID support and leadership in private health sector programming, with an emphasis on exploring and advancing private sector innovations.
 The core goals of SHOPS Project Kenya are:
(1) Expand access to priority health services (Reproductive Health/Family Planning, Maternal and Child Health, HIV/AIDS) provided by the private health sector in Kenya; and
(2) Increase the number of Kenyans insured through coverage for priority health services.
The SHOPS/Kenya program is centered on three main technical component areas aimed to:
(1) Strengthen the enabling environment for increased health care access in the private sector;
(2) Identify, expand and improve financing options for increased private sector health care coverage; and Initiate, implement, and scale up innovative and sustainable private sector health models for improved service delivery.

 The Consultant will be responsible for the following activities:Prepare for and conduct interviews with the SHOPS Project Kenya to understand the assignment and expected deliverables below.Prepare for and conduct interviews with the insurance provider to understand their expectations on the assignment.Review the insurance provider’s existing IEC materials, marketing and distribution strategies for their existing products from which best practices and lessons can be derived for the development of the proposed materials.Review existing IEC materials produced for insurance in other contexts.Review existing training modules used for consumer and provider education on insurance.Utilize the information gathered to develop a communication approach for the desired behavior change and present this to SHOPS Project Kenya and the insurance provider for approval.Develop the IEC marketing materials and training modules in line with the communication approach that will target 2 groups:1. Potential customers. This group represents the potential market for purchasing health insurance. IEC materials targeting this group will focus on consumer education for general consumers and targeted marketing through distribution channels. These IEC materials will instill an understanding of insurance, communicate the value proposition of the insurer’s product and will use a variety of delivery channels as identified in the desktop review and interviews, including but not limited to, print, radio and inter-personal communication through peer-to-peer networks.
2. The insurer’s partner healthcare providers. The training modules will focus on training the insurer’s staff on how to train healthcare providers on key insurance concepts and how to implement the insurance scheme from the perspective of the healthcare provider.Training modules for healthcare provider education will instill an understanding of insurance, communicate the value proposition and administration of the insurer’s product and will use a variety of delivery channels, including but not limited to, e-learning modules and classroom modules for use by the insurer’s agents.Prepare for and present to SHOPS Project Kenya and the insurance provider the drafts of the materials developed and proposed training modules; and incorporate their input and comments to the materialsReview the final draft and present to SHOPS Project Kenya and the insurer.Deliverables and Period of PerformanceThe consultancy is expected to run from 25th February 2013 – 31st May 2013. The following deliverables are expected from this consultancy, with limited flexibility on the deadlines:
1. Meeting notes of all client interviews (For the duration of the contract)

Scope of Work for Operations Project Support Staff

Background


Strengthening Health Outcomes through the Private Sector (SHOPS) Project is a global five-year USAID-funded project, which aims to expand the role of the private sector, including NGOs and for-profit entities in the sustainable provision and use of high-quality health information, products and services.
SHOPS Project Kenya is partnering with GIZ to conduct a national health care costing study to inform payment mechanisms in both the private and public sector. SHOPS will engage an Operations Project Support Staff to support the logistics and coordination of the costing study.

Scope of Work

The Operations Project Support Staff will be responsible for the following Scope of Work (SOW).Prepare the logistics, including invitations, confirmations and venue; and take minutes at the bi-monthly internal management meetings during the duration of the DCM studyPrepare the logistics, including invitations, confirmation and, venue; and support administration of all the stakeholder consultation workshops Prepare the logistics for, including invitations, confirmations and venue; and rapporteur at the following meetingsreview of the data collection tools, data analysis and reporting framework sampling of facilitiesinternal workshop to finalize all the toolsPrepare logistics for the classroom and field training of data collectors Prepare logistics for the data collectors work in the field and data entry/transfer to the DCM workbook by the StatisticianAssist the Finance and Administration Manager in procurement of the DCM equipment as well as the assignment, management, and retrieval of the equipment with the research assistantsAssist in processing invoices and payments for the vendors and consultants contracted during the studyA higher diploma or degree from a recognised Training College or University Over 2 years’ experience in administration duties and logistics supportExperience in a similar role will be an added advantageAbility to work in a teamExcellent communication skills with ability to express ideas clearly, logically and effectively, both orally and in writing  Fluent spoken and written English Computer literacy and ability to use MS Office software

General Job Responsibilities

A Technical Specialist (TS) at this level would be an expert in the professional field.  

He/she would have direct project management responsibility for a small project or one component of an overall project that is large, highly complex, highly visible, and well-funded.  General responsibilities may include:Provide direct  management and technical direction to one or several components of the project;Design and implement one or more of the project’s technical aspects;Manage and monitor technical aspects of the program, and advise the Project Director on technical, budgetary, and management aspects of implementation;Provide research, analysis, and consultation on project planning, implementation, monitoring, and evaluation;Collaborate with key partners (government, NGO, commercial sector, USAID partners and other donors); andServe as subject matter expert.Specific Job Responsibilities
The Health Finance Specialist will report to the Chief of Party and liaise with technical management at Abt/SHOPS headquarters. The Health Finance Specialist is expected to:Provide technical assistance (TA) for health finance activities, including research, analysis, and advisory services.  These activities may include but are not limited to: identification and scale-up of health care financing mechanisms, including micro-insurance and medical savings accounts; development and piloting of provider payment mechanisms; expansion of health insurance coverage through promotion of healthcare financing partnerships; and costing analysis for outpatient and inpatient packages.In collaboration with the Kenya/SHOPS team, work to develop and sustain partnerships with government partners, private sector stakeholders, and other donors in support of these activities.Provide management and budgetary oversight on implementation of health financing activities to ensure all activities are completed within the agreed to timeframe and budget.Track progress of health financing activities against the workplan in accordance with available funding.  Ensure all processes and procedures for project implementation are in accordance with USAID guidelines and protocols.Support monitoring and evaluation of project activities, including ongoing assessment of health finance activities, and support project documentation (including financial reporting) as needed.  Contribute to project deliverables, including reporting and writing.  As additional funding becomes available, provide guidance for additional activities or expanded scope or existing activities in consultation with the Kenya/SHOPS team and USAID/Kenya.  As a member of the Kenya/SHOPS team, participate in and contribute to the workplan development and updating process, including providing technical and budgetary guidance for health finance activities.  Participate in team meetings and contribute to the overall success of program implementation.  Develop SOWs and help identify consultants to support program implementation.Regularly track and report progress of health finance activities by contributing to quarterly, semi-annual and annual reports in accordance to USAID reporting guidelines.Contribute to business development efforts in Kenya as appropriate, by providing technical input into new proposals for USAID and other donors.Master’s degree and 6+ years of relevant experience OR the equivalent combination of education and experience;5+ years of experience in developing and managing health finance programs for vulnerable populations, preferably in Kenya; Experience working on USAID programs or other donor funded work; Excellent partnership development and collaboration skills, including with Ministries of Health, private sector stakeholders, and other donors;Project management experience dealing with various elements of a large project and various team members across the world;Demonstrated teamwork skills and ability to produce high quality work in a timely manner;English language fluency and excellent written and spoken communication skills;Computer literacy.Preferred Skills and Qualifications:Technical expertise in one or more of the following areas: health economics, health financing, health insurance, and costing.Familiarity with the public sector’s mechanisms and policy/regulatory environment related to health financing in Kenya.Please include a short (3-5 page) writing sample.
